Rare!
1. Mg-containing laxatives, antacids, or enemas in patients with renal failure (all are contraindicated in this group of patients)

2. MgSO 4 over-infusion during eclampsia treatment

3. Less common, but noteworthy, causes: tumour lysis syndrome, milk-alkali syndrome, lithium overdose, Epsom salts ingestion (even as a gargle)
